Attendant Passes
An Attendant Pass is a card that is issued to a person living with a disability that requires the support of another person (or persons) to be able to access events and activities in their community. This need has been verified by a professional during the application process.
The pass enables an attendant or support person to accompany a person requiring support to activities and/or events for free or at a discounted rate determined by the accepting venues.
The attendant pass belongs to the person who requires assistance.
Inclusive Services can issue an Attendant Pass for staff of agencies or organizations who support individuals requiring support. Contact us to learn more about the attendant pass by calling 709-576-4556 or email inclusion@stjohns.ca.
Apply
Complete the application or attendant pass renewal application in the name of the person requiring support. A medical professional, social worker or representative of a disability agency must verify the request. Please include a photo.
Submit the application in one of the following ways:
